Get rid of the dirty tenants, get two clean tenants. It's kind of surprising the good tenants haven't been driven away.  Regardless of the type of roach, the first step is good hygiene, and then treat and treat and treat until they are gone.

In my area, I don't have seperate water meters, so I have to eat the cost of water and sewer. Eventually I want to get individual meters, and have the tenant be somewhat responsible for the ammount of water they consume. Tenants are responsible for gas and electric, and I have been having them pay the trash bill. When i have a vacancy I can schedule "show and clean" electrical service from socal edison. If a tenant left a big balance, they just ask for proof you aren't the tenant (like ssn or ID)

I'm not sure if Termites are an issue in PA...but in my State, termites are a big problem. I would make sure I have a termite guy out to inspect, and also have a roofer check out the roof, not just the general inspection guy. Those two things can bite you in the *** big, they are expensive. 
Ask for actual tax records on rents collected...dont accept "pro forma" operating statements, those are wishful thinking.

My PM contract has similar verbiage. I also have to name them on my insurance policy

The order of items is correct in the list you have. I have occaisionally found tenants during renovation....Obviously you cant put pictures up, but you can put up a sign, or advertise the place is gonna be available so you can applications coming in. 

I have a no sublet clause in my leases. I also check air bnb (and smiliar) to see if my apartments show up...they never have, so far.

I have a great PM. I don't know how the hell they manage to do all they, and pay all their staff, keep all of our tenants happy...and still turn a profit. I don't know how you would start a PM company with no clients, and build up the roster...it would be tough.

No. Then they will have all kinds of silly favors and requests of you. "Well you let me last time..."

I'd absolutely charge her for the key. I've been lucky when someone leaves without returning the key...they usually leave a window open, and i get in and change the locks myself, without paying a locksmith.

I would get a kick out of a prospective tenant trying to haggle on price. It hasn't happened to me yet, but I would really wish I would.
