I agree with Andrew! Life happens and this may happen periodically but shouldn't be the norm.

We require it and obtain proof during lease signing or renewal. Now if they cancel it I would have no visibility into that but every year I ask for proof that they are carrying it. 

This seems to be common at least where I am at. Every bank that I have spoken to about getting a loan through my LLC has requested the operating agreement. In addition, we also had to provide it to the bank just to open our accounts.

I just found a portfolio lender in my area that I am meeting with on Friday. They exist even in smaller areas like where I am at. Give the bank a call but make sure you bypass whomever answers the phone as they typically will have no idea what you are talking about.
