Chicago - Cash for Keys Form

I have an MF contingent and I want the tenant out before closing.  Do you have a cash for keys form I could use in Chicago, please?  I will send this to the Seller for consideration.  If not, this deal is likely dead.   I will try negotiating relocation, paying for movers, and paying for the cash for keys portion.
