Can anyone recommend a bank/contact at a bank to refinance a primary residence? We have a 2-on-1 in San Diego.

His fees are not the lowest but I recommend Brett Mills at Casa Financial.  I once tried to go with a cheaper option and they failed to get me the loan.  Failing to get that loan on first attempt cost us a lot of money.  Brett got me that loan and I have no plan to look into cheaper options again.  IMO he deserves the fees he charges.
