Best way to inform tenant that you are not renewing their lease?

I have a tenant that is chronically late on her rent payments.  Often she will send in a check and 10 days later I will receive information that there are Non-Sufficent funds in the account.  The check will bounce multiple times and I continue to hound her all month long to pay.  Eventually, she does pay the rent and the late fees, but I'm done with this dance.

What is the best way and how much notice do I need to give her that her lease will not be re-newed?  I was looking for a form on this website but didn't see anything.  

It's not an eviction, and it's not a 3 days notice type of situation.  I'm just NOT going to re-new her lease and she will need to move on by end of August.  I also don't want her to get upset and trash my rental either. 

Does someone have a letter or has a method that they use to politely ask their tenants to move out?
