I need a eviction form for Arkansas

I have a house in Arkansas and the tenants have not paid since December. They will not return my phone calls. I have a trust worthy person that will post the eviction notice for me. I live in Texas at the present or I would of already started the eviction. I also need to sent a Limited Power of Attorney to do the eviction that only gives my stand in the right to evict and/or collect rents on subject property.
1. Could someone that has a Arkansas specific eviction form email it to me?
2. Could someone send me a LPA form that they would use for evictions only.

I already looked on line and all I found was bait and switch sites, so I'm asking for more help than some wise guy telling me to look online or talk with a attorney.
