Im looking for any suggestions as to the direction i could go with for rehabbing and selling to use the proceeds to buy more or rehabbing and living there to use a heloc to purchase more. 

There is a big difference between "yeah that sounds good" vs here is a stamp, you may proceed with your plans.

That's just how the title co. is required to report the distribution of funds.....just like the proceeds from a sale.

Is there any tax issues on the LLC side if I transfer the property back as an equity withdrawal to my name from the LLC then proceed with the sale?

I have to consult with Consumer Protection Agency  folks to find out if I can deduct the unpaid utilities from their deposit but I think it is illegal to disconnect the utilities. 

Many lenders have their own forms, but you can start the process with almost anything you find on the Internet.You should submit the estimated settlement sheet showing net proceeds to lender, with nothing to borrower.
