I've been listening to BP for the past 6 months or so after a colleague told me about the podcast.

3) My thoughts these past few days have been that an annual PM inspection in Jan 2016, 2017 and 2018 could have remedied the issues above and the response from the PM is pretty weak. 

Took our PM inspector until this past Tuesday to do an initial inspection because she kept making excuses to not let anyone in.
