From an operations perspective, not too much changes.

A thousand things can derail a flip - changing market, underestimate expenses or overestimating income, mistakes, errors, contractor issues, permitting issues, just to name a few. 

If so, underwrite it that way and then after you buy it and have a contractor consider changing the plan to add a unit if that makes more sense.

fair enough, but If you're saying it's hard to pay the bills, and you don't know what you're doing those two things need to change real quick if you want to have a chance.Owner-occupant loans are a great way to start acquiring property for a low barrier to entry; that is most of the advantage.
