@Jeff Cliff  Best practice is to have all three on the lease and have some sort of language in the lease that they are all jointly and severally liable for performance of all the terms of the lease, including the rent payment.

While I understand that this language does not explicitly call out the fact that there is an extra guest fee; the reason you did not see the extra guest fee when you booked is that your booking request was for 1 adult only, not for 5 guests.
