Your PM is in a position to monitor this and handle it, if necessary.

Part of the purpose of inspections is to monitor tenant activity to protect your investment from this very type of situation. 

With the work-flow that you outlined, do you have a system to monitor the states of maintenance requests so that you know what you or other people on your team need to focus on?

If the lease says the Tenant is responsible for utility bills, then pass that charge on to them.If the Property Manager was responsible for monitoring the water bill to ensure it was paid, then you can also demand that they compensate you and then let them go after the Tenant for compensation.
