The actual insurance premiums in PA are regulated by the state with some exceptions (most notably approved attorneys).

If the houses are worth something, you can always sell the tax deeds and avoid the legal costs.  

Does anyone know how this is defined legally in CA?

One more question: If I approved the 20 year old with a co-signer and then allowed the 17-year-old to be listed as an occupant, would it make sense to have him sign as a tenant when he turns 18 in 5 months?

You don't want to get stuck with negative cash-flow because you can't find another care facility tenant to rent it.Also, probably want to check with your insurance provider and see if all those tenants who are likely to fall and break a hip are going to be an issue for them, and look into the legality of what happens in the care facility owner dies and you're stuck with 5-8 tenants in the home with no one to care for them and no living descendants.
