As @james wise mentioned, Ohio you have to sit in a brick and mortar location, and endure long drug out sessions to get the required hours in.  

Usually it's any closing costs you endure on any regular closing such as transfer taxes and doc stamps etc 

Different filing codes...maybe different formats, but with a little sleuthing it is something you can figure it out.Your second qq is a whole other animal.
