Help with MI eviction paperwork

Hello!

I'm doing my first eviction and have been reading as much as I can. After mailing the tenant the 7 day letter I now need to file for eviction with the court.

I called the magistrate and she talked about dc100a forms. She said I need to mail four or five of them to the courthouse in envelopes then need to mail five copies of another letter somewhere.

Can anyone please give me clarification about this? I'm hoping to do this myself for the learning experience without a lawyer.

This is in Durand MI in Shiwassee county.

Also, the tenant was supposed to pay utilities and isn't. How do I add next month's utilities to the eviction lawsuit?
