Targeting Big area vs small area

What do you think would have the biggest impact and return: 

1 - Sending out direct mail to a large area (about 1/3 of 3.5 million population), once or twice?

2 - Sending out direct mail, repeatedly to a smaller area?

Tell me your opinion and/or experience with that
