Tenant Application Forms

This may have already been asked but could no find in my search. What kind of forms do you include in your application packages? It is getting harder and harder to get any kind of verbal references anymore. So I need to get something that gives me authorization. I am using outside resources to pull credit and background checks and letting the tenant pay for this service. It makes it a lot easier to weed out the not so serious people. But I was thinking of charging an application fee and doing these myself since I cannot seem to get the co-applicants to complete in a timely manner. 

I only have one rental house if that makes a difference.

Any help or suggestions appreciated

@Martin Qualls There are platforms that allow you to create custom rental applications with additional questions. You can also require your applicants to upload files like pay stubs or bank statements.

Many also allow you to charge an additional application fee like you mentioned on top of the screening charge. Application services have really taken a huge leap in recent years.
