When denying an applicant, what form or forms do you send to them?

You need to explain why they're rejected, in general, non-discriminatory terms. As 

@John Johnson stated, you should have published your rental criteria when you listed the property for rent.  Your rejection must be based upon failure to meet the rental criteria, which themselves must be in conformance with state and federal law, and local ordinances.    And yes!  Be careful with your wording.
