TV Wall Mount - Release of Liability Waiver

I have a TV wall mount installed for the convenience of my tenants and worry that one day, a tenant may either mount their tv incorrectly or do something by accident to cause their TV to fall off the wall. Would it be appropriate to make the tenant sign a release of liability form prior to using the TV wall mount? If so, any tips on what language to include? Thank you

One of my favorite pics.....& yes we saw something similar when they put an older heavy plasma on the existing TV rack someone else had previously installed. During a rehab I always imbed 3/4 plywood behind the drywall, between the studs. Young kids my grandkids included do have a habit of pulling on the TV itself, so precautions are imperative.
