Tenant Name Change - Lease Addendum

I have tenant who is in the process of changing their last name back to their maiden name. What is required here to update the current lease with an addendum? Does anyone know where to find a template for this?

Her lease is ending Jan 31st and she’s most likely renewing for another year. Obviously, I’ll get a copy of her updated driver’s license to confirm the name. Are any other documents necessary?
