Transfer of Lease to PM company

I just acquired a property in Ohio. My real estate agent will be placing tenants for me and then I will transfer over to use a property management company sometime soon. I do not have a company picked out yet. This was the easiest option for me at the moment and my agent has helped me so much that I want to give her the tenant placement fee.

How should the lease be set up to best transfer it over to a property management company in the near future?

Who's name should be on the original lease, mine or my agents?
