tenant move-out questionnaire??

i have owned my first rental for a little over 2 years. Its a 3 unit, we filled one vacancy after a rehab and the other two units still have tenants from the old owner.  We have been informed that one of those tenants will not be renewing their lease and moving out at the end of March.  I am wondering if anyone has any type of questionnaire for tenant move out? what kind of questions do you ask?

When I have a tenant leaving I send a letter that outlines what to expect at move out.  Things we note are what's expected in cleaning, that the property may be shown to prospective tenants, checkout time, key return process, reminder to forward mail, notify utility companies, etc.  WE include a copy of the move-in form they signed at the start of the lease and state that it will be used as a guide to determine the move-out condition. 

A questionnaire is good.  I usually have a conversation with everyone during the move out inspection to find out why the decided to move (if I don't yet know), where they are going to, what they thought of the place, any "quirks" that someone living there might know about and get used to but that I might not notice....like you have to sometimes jiggle the 3-way light switch to get it to catch...stuff like that.
