Rent and Possession Question

Hello, I had a tenant I inherited, who left at the end of his lease, and still owes for last 2 months rent.  The security deposit covered damages.  My question is does someone have a site or forms they can share so I can file, pro se, the Rent & Possession Judgement in St. Louis city?

I read that I need to file and affidavit, petition, non-military affidavit, and motion for appointment of special process server.  Unfortunately, I have not yet found online forms for these.  Any help would be appreciated.
