Letter of denial due to credit when using Cozy to screen tenants?

I am using Cozy for people to apply to our rental, and we get the credit and background checks through Cozy. I know we have to send a letter when denying an application due to credit, but I can't find anything on Cozy's site that has a sample denial letter. Anyone know where to get a good copy of a denial letter? I believe the letter has to state what company we used to get their credit report, but not sure how it should be worded since we got the report through Cozy.
