Inherited tenant is reluctant to have things fixed

Two family house that we live in purchased July 2015 we inherited the tenant. She has been here for 7 years pays her rent normally two days late and she is slow to notify of any repairs. The bathroom floor needs to be redone the tile is cracked and by my standards unacceptable. We have tried on numerous occasions to have the worked done but she is never available or doesn't answer the door. The last time I was in the apartment October 2015 it was a mess. She isn't very clean. What can we do? I don't want the floors to create another problem.

What are the requirements under NY State tenancy legislation?

Here we are required to provide the tenant with 7-days notice of entry to affect repairs.  Naturally, we first try to schedule a time with the tenant, but if there is no cooperation or response, we serve notice and come and fix the issue.  The tenant is not required to be present (it's usually preferable if they are out).
