Move Out Cost Sheet

I've read from several people on here of doing a move out cost spreadsheet that is to be signed by both parties before moving in. 

For example, if you leave trash in the unit it will cost you this much. A fork in the drawer will cost this much? 

My questions: what type of items are included in this list? How far do you go with the requests? How severe are the costs?

I checked the Fileplace and could not find an example. If anyone were to provide that I would obviously appreciate it greatly. However, any feedback would also be appreciated.

I know this is an old post but just in case anyone needs the info, there is a list like this in Mike Butler's book, Landlording on Autopilot. He calls it a move out price list and he claims it is a downloadable form if you buy his book - although I didn't see it in the downloaded files I got from him. Anyhow, in my copy of the book it is on page 227. Hope this helps someone out there!
