Advice Needed -- Tenant Not Paying, Agrees to Vacate

Hi there,

I have a tenant who has not paid this past month's rent and after sending them a formal 5 day notice, they agreed to vacate the premises at the end of the month so long as I do not file for a formal eviction. 

I really don't want to involve the courts or attorneys unless absolutely necessary due to cost and time. Instead, I'm wondering if there is a legal template available that the tenant can sign to waive their rights and agree to vacate at the end of the month. I'm awaiting on my attorney to respond but thought I'd reach out to the community as well.

Appreciate any feedback.
