Wording for a notice to move for full renovation
I have a 30 plus year tenant that can not keep up with maintaining their unit, it is filthy inside and in desperate need of renovation, all systems are in good repair and I respond very quickly when issues are brought to my attention.
It is also well below market rent, and they struggle to pay the current rate.
Looking for suggestions on how to gently proceed with the notification to move.

First, you need to realize this is a business relationship. It's not the same as kicking your own mother out onto the streets. Give them a short, professional notice and move on.
Second, I don't think a specific form is required in Massachusetts. Type of a short letter, deliver it by hand, and move forward. Since they've lived there so long, I recommend giving them 60 days to move as a means of softening the blow.
Third, don't negotiate or accept excuses. If they tenant is $300 below market, that's costing you $3,600 a year. Tell them you are losing money and can't afford to let it continue. Then stick to your guns.
