Statement of claim. Eviction/unlawful detainer Jefferson county

Tenant stopped paying rent. Posted the Notice of Right to Cure and Termination Notice on Sept 4. Of course, no response. 

So I need to file the Statement of Claim, and have questions about the form and process.

Do I complete the form, have it served to the tenant by personal service, then file it with the court?  I believe that is the fastest way and easiest way to get a judgment too, correct?
