Contract Addendum Question

Does anyone know of a good example to use to write up an addendum to a contract? I need to write an addendum basically just to state that an occupancy agreement will be done at closing but the original contract states that there would be an addendum as I was unsure what process we would use for that at the time.

Other than the standard realtor blank addendums, you could just create your own.

ADDENDUM
contract date, property, between______buyer and _______seller
Parties agree to following changes.............
All other provisions remain the same.
Sign, date.
