How to properly put a lien on a property?

Hi guys,

What is the proper way to put a lien on a property?

Not mechanical lien.. Regular lien.

Lets say I lend a 100K to someone and they give me their paid off property as a collateral.

What is the proper way to do it to make sure its executed correctly?

what kind of document should be used? Warranty deed? Deed of trust?

If the property is in LLC, should the lien be against that LLC? Or against the property ( put a physical address)

Should it be recorded at the assessors office?
