Need a mortgage template and tips on filing

I am borrowing funds from my father in law to purchase and rehab a rental.  I am looking for a simple mortgage template to fill out the necessary details so I can go get it filed with Tulsa county.  Worst case scenario, I could always just manually retype the last mortgage that was filed by a bank and make changes as needed but that is a pain in the butt without even contemplating possible copyright issues.

Simple terms:  $80,000 at 5%, interest only and paid quarterly with 2 year term and balloon.  There is no prepayment penalty.  Plan is to get property rehabbed and after necessary seasoning, seek permanent financing via conventional route and obviously repay him.   Rinse and repeat with future purchases.

Any tips or advice from those that do this?  This is obviously my first time doing this myself.  All past mortgages have been handled by banks and title companies.  Anything special that needs to be done to make sure this is all legit?  Show his interest earned, deduct my interest paid etc etc.
