how to take a previous tenant to small claims without their current address

Summary: how do you take a previous tenant to small claims court if you don't know her exact current address?

I'm learning some lessons with my 2nd tenant. She moved out earlier this year. She violated the lease twice so I kept the first security deposit and she ended up breaking the lease before I could collect a 2nd security deposit for violating the lease the 2nd time. But she agreed to a payment plan. The day she moved out in Feb. I had her sign a re-payment agreement which stated she would start making payments to me for $50 a month starting in April. The agreement stated that if she was ever more than 5 days late or never started payments she would be in breach of the agreement and I would take her to small claims court. I've been waiting on the funds to take her to court so I was letting it go for now.

She recently called me about 3 weeks ago out of the blue from the same number she had when she was my tenant and said she was filing for bankruptcy and would be able to start paying me in November. So I created a new re-payment agreement. I signed it and mailed her 2 copies, one for her to keep and one to mail back to me.

As of today, she hasn't mailed it back. I tried to call her and her # is now disconnected. I see on Facebook she moved back to the same town as my rental is in. 

So how do I sue her for the security deposit if I don't know her exact address?  And since she is out of state and obviously in a different county, will all court proceedings occur where she lives as opposed to where I live?
