HELP! Homeowner is in a nursing home, what next??

Hey BP family,

So there's this vacant property in my neighborhood that I have my eye on. The house has been vacant for a few years as the homeowner was diagnosed with dementia and is currently in a nursing home. I checked my county's tax assesor site and it still states that the property is the homeowner's primary residence. My question is, who do I reach out to about the property and how in the world do I find them? Thanks in advance for any advice :)
