Looking for some blank forms

Hi folks. I am closing on my first sfh on Monday. I have found a GC for the project. I have been told that I need several forms for GC to sign. I am missing 2:

1) Insurance indemnification form

2) a good lien waver for GC to sign before receiving final payment

Any help would be appreciated!!
