How do you send an offer?

Do you send an offer written in paper or do you use a contract or what do you use to send an offer to a seller represented by a realtor?

If there's a realtor involved, you can get your own realtor to help represent you and they can write the offer. Conversely, you can put the offer in with their agent. Offers are usually in writing as they're a "contract" and not to be made lightly. Sometimes I'll have my realtor make a "verbal" offer to the listing agent to see if our proposed offer is in the ball park. If there's already 3 offers higher than ours, it would be a waste of time to submit a lower one. It's in the offer that any inspection contingencies or anything else needs to be addressed. If you don't include it then, don't assume you can add something later.
