MLS Access as an unlicensed real estate assistant in Utah

Hi, I'm just starting out in REI and I have a real estate agent who is willing to let me be his unlicensed real estate assistant in Utah. I heard there is a form that we need to fill out and send to our local realtor board in order for me to get my own login to the MLS. Does anyone know where I can find this form or if this is even true? I went on to the utah realtor board website and there is a link that says "forms" but I'm unable to access it without being an agent/member. Is this where I can obtain the form? Thanks in advance.
