That level of debt is completely understandable for an orthodontist. 4 years of college, 4 years of dental school plus 2 years orthodontics fellowship tuition and living expenses adds up.

For us they’re called either a GPR (General Practice Residency) or AEGD (Advanced Education in General Dentistry), and then of course there are specialties (orthodontics, oral surgery, etc).
