The FSBO from Step 1 took blurry cell phone pictures at night time and included photos of family pictures, clothes (not the closet with clothes hanging, but actual photos of close-ups of shirts), and had pets and people in pictures.

You could get the health department to do a walk through and write the changes that must be made, then give the hoarder a 5 day notice to comply and stay in compliance.

I can't speak specific to cap rates today but when I was negotiating new builds for the health system I was overseeing it was right around a 10 Cap but that was about 3-4 years ago, specialty clinics and these were new builds.
