The first 10 years of the HELOC terms are usually interest only payments, meaning smaller payments but it only pays towards the interest and not pay down the loan, but check with the bank that is offering the HELOC, because they have many ways of doing a promotion

I can't speak specific to cap rates today but when I was negotiating new builds for the health system I was overseeing it was right around a 10 Cap but that was about 3-4 years ago, specialty clinics and these were new builds.
