The insurance co faxed me the following (and is mailing a hard copy for my records): Type of Insurance: General Liability there is a X in the box for Commercial General Liability, and a X next to a box that has Occur (not sure what that means).

Some people use P.O. boxes for getting their mail - so they would also fit into the same situation you describe, where the address for the loan and the address for the mailing purposes differ.
