They may be fine now, but in the summer the heat gets through the first layer of shingles and the second layer will eventually curl, causing flat pockets of water to form that will eventually leak down through and destroy the sheathing and the rafters/drywall underneath.

Is there a basic form I can use that would cover all of that?

For me to consider buying out of state property I have the ability to be able to drive or fly there if needed.Could rent out your current house than move to another live in it for the required period of time than turn that into rental as well.Figure out what your long term goals are of and work backwards  from there.

Rents are secure in multi unit blue collar and for me following the last dip in 2008 I still raised rents annually.
