Banks (at least in Texas) generally require the certificate of formation, certificate of filing, the EIN#, and the LLC Operating Agreement listing the members that have the rights to be able to open an account and withdraw money from the account.

. - just legal BS but may save you personally in a law suit, you are mixing personal gaurantee with business and breaking your LLC protection. here is NJ permit for the owner section, notice how each section starts with "I":CERTIFICATION IN LIEU OF OATH I.
