@Scott Schaecher, my personal favorite was when they moved the washer and dryer to the middle of the basement for the first floor unit and just painted the concrete floor around them, so when we moved it back there was an unpainted rectangle in the middle of the room.

(In fact, this is a code requirement pretty much anytime the WH is on a subfloor that isn't concrete on grade... and even then I say to get one.)Drain pans are the cheapest insurance you can buy with a WH.

The fiberglass one that I ripped out had been there over 20 years but had been installed over an uneven concrete floor with voids under the pan so it had cracked and bondo repairs weren't holding.
