Unlike most of the country, her locality requires permits for even renting out a single family home.While annoying, I decide to do the right thing and try and get the permit, even though everyone I talk to says not to bother since nobody else ever does.In order to get the permit, the town needs to inspect the property and review the Certificate of Occupancy and a land survey, so I file a Freedom of Information Request to get those documents from the town (which is a whole other ordeal).

Keep a photocopy of the notice and send it with a “certificate of mailing” so you have proof you sent the notice.

The form can require a signature and identifying/licensing information and place of business of the medical professional or social worker that completes it; and you can require that it be provided by a "reliable source" (internet certificates, out of state professionals (unless prospect has relocated, but has personally been seen within one year) or similar legitimate, verifiable sources.

But now we have certification from the state inspector and laboratory that the tank has been removed and the soil is OK.
