I would replace the toilets/flooring in the bathrooms, add a new vanity, and just fix and resurface the tile and tub for showers along with new fixtures. 

So between his health problems and the no-work situation he can't pay the total rent.

It would merely be like adding an additional 'feature' to the listing like a hot tub or pool in the end.
