The form can require a signature and identifying/licensing information and place of business of the medical professional or social worker that completes it; and you can require that it be provided by a "reliable source" (internet certificates, out of state professionals (unless prospect has relocated, but has personally been seen within one year) or similar legitimate, verifiable sources.

hmm; In Calif, one holding only the Agent certificate must be attached to a BROKER.In that case, using the MLS behind the broker's back is a conflict of interest and can lead to a revocation of your license.
