My dilemma is that it's a non MLS property from a wholesaler that has a closing date of a couple weeks, hence the reason for a HML.But, you are saying that it is possible to refinance an existing loan using an FHA 203k loan. 

To be a landlord I’d get a counseling / mental health degree to deal with insane irrational tenants which is 99% of them .

You’ll have expenses tied up with sending a handyman to the property to address an “emergency” health issue only to come to find out it was a cleaning issue.

We had an issue come up on our primary residence during refinancing. The property appraised for less than last year, the year of purchase, such that we’d have to put in about $33k at closing.

It is the same play they did with health care ...and we know how well that worked..  

I wish I could have figured out a solution to this dilemma that would have led to all parties walking away pleased.

I really hate to say this, because I have a very soft spot in my heart for the elderly and the disabled (and that includes mental health issues); but, you don't want to make a commitment like that and then have to deal with a lengthy eviction process which can take a turn in any direction since the tenant is a reported schizophrenic.
