I used my own home address. 

I was confused but when I looked at the "Owner's address" they had separate addresses such as California, Georgia, The Keys.. 

To address the fee %, I later read the 6% recommend after posting, so I will ensure that for the future.

I need the actual report because it shows prior addresses, mortgages, etc. that confirms info. on the tenant's application.
