That way when you go to court for an eventual eviction you have everything backing you up showing he has been non-compliant and if he really wasn't impacted by COVID that will be the nail in his coffin.Sorry for the long response, just trying to help from someone who's in a highly pro-tenant state.

They also came back and had to lower our max LTV and that was the nail in the coffin.

I don't believe a permit was ever pulled for this.I was going to fix it up to make it safe for use again (it has exposed nail points on the underside of the roof, and the floor is sagging in some areas).However, what I'm wondering is: even if I fix it up, and a tenant gets injured in there anyway (trips and falls) and sues me, will my insurance deny liability coverage anyway, if they find out it doesn't have a permit?

Yeah, it's a bit of expense, but they don't have to worry about getting the windows covered up when they move in and I don't have to worry about someone nailing junky sheets over my windows.I do save the extra slats for repairs.
