A person might be a good teacher, but a poor practitioner (book smart, but not street smart in a sense).  

(I doubt they have health insurance and looking at their teeth, dental is probably a vast mystery to them) I purchase workman's comp.

That level of debt is completely understandable for an orthodontist.  4 years of college, 4 years of dental school plus 2 years orthodontics fellowship tuition and living expenses adds up.
